Immunizations are required for students to register and attend school in Arizona (A.R.S 15-871 & 15-874). Below is a link to the Arizona School Immunization Requirements for Kindergarten through 12th grade.  Please review this form and make sure your child has the appropriate immunizations for his/her age and grade.

I will review all records and if your child is missing any immunizations required for school, I will contact you and let you know what is needed.  As a reminder, there are immunizations required for students who are between 11 and 12 years old.  You will be allowed a grace period after receiving a notice of missing immunizations to get the necessary shots.  After the grace period, if you have not gotten your child's immunizations up to date, s/he will not be allowed to attend school until the required shots are received.  After completing the required immunizations, please provide me with a copy of the proof of immunizations so that I may update your student's record.  You can scan and email them, mail them, or drop them off in the office.
