• The Highland Lakes Health Center does not keep medications on hand for students. If you'd like for your student to be able to have an over the counter medication (Tylenol, Advil, Tums, cough drops, etc) during the school day, it will need to be provided to me in its original container. I will also need the following form completed by you. By providing the medication and the form, you are granting me your permission to give your child that medication.  Medication Consent Form

    If your child is in need of prescription medication (a medication given to your child by their doctor), I will need the Medication Consent Form above AND this form (Prescription Medication Doctor Form) which is filled out by your child's doctor. This includes short term antibiotics needed for an ear infection, for example.
