Register My Athlete (Aktivate)
All athletes must create an account at Register My Athlete (Aktivate). Once you create an account you will find all physical forms needed. Be sure to complete all items in order to be eligible for your sport. You will receive an email when you are cleared for tryouts. If documents are unable to be accepted there will be a note as to why and the document must be uploaded with the correct information. Brainbook Concussion Course
All athletes are required to complete the Brainbook Concussion course and upload the certificate of completion to your Register My Athlete account. This is a one-time requirement and does not need to be repeated each year. You can find that course . Register as a Boulder Creek Student.
Dr. Physical Forms
These forms must be completed by your child's doctor after their physical examination. Once completed, please upload it to the RMA site.
