    Arizona School Immunization Laws 

     
    According to  (ARS) §15-872 (B), "a pupil shall not be allowed to attend school without submitting documentary proof to the school administrator unless the pupil is exempted from immunization pursuant to section 15-873".Students of all ages are required to have immunizations to be enrolled in school.  
     
    Pre-school, kindergarten and 6th grade have very specific immunization requirements.  Please visit the azdhs.gov website and look under immunizations for additional information that outlines age and grade-specific .  It is required that the parent/guardian submit an up-to-date immunization record or a valid ADHS exemption form to their child's school, preschool, or child care prior to the child's start date.
     
    Exemption forms are available in the Nurse's Office. There is a Religious Exemption for pre-school, a Personal Exemption for elementary age, and a medical exemption form that must be filled out by a physician if a student has a medical condition that prevents the child from receiving vaccines. If an illness breaks out and your child is not immunized, they may be asked to remain out of school for up to a three-week period.
