• Allergy, Asthma, Diabetes, Seizure Action Plans

    If your student has a chronic illness please inform the School Nurse before the school year begins. If the student must take medication you will need to have a Medical Authorization to Administer Medication Form filled out & signed by your Health Care Provider. Any prescription drug must be brought to school in the ORIGINAL prescription bottle with the label on the outside including student's name, medication instructions, and doctor's name along with a completed Medication Log for the current school year.

    If your student needs to carry insulin, an inhaler, EpiPen, an antihistamine, or seizure medication in their backpack while on campus, the parent/guardian must sign a Self-Carry (Consent to Carry) form at the Health Center AND fill out an Action Plan for that student's particular medical condition.  This way, we know how you would like us to treat your student's health condition here at school.

    Allergies

    If your student has severe allergies (food, nuts, seasonal, etc.) that require medication, please complete an Anaphylaxis Emergency Action Plan form and bring the any necessary medication to the Health Center. Students are not allowed to carry any medication on themselves without prior approval and documentation.

    **We will need the original prescription box for any Epi-Pen, asthma inhaler, or nebulizer treatment.**

    Asthma

    If your child has Asthma, please complete a Student Asthma Action Plan and bring the prescribed inhaler and box with the prescription label attached to the School Nurse.  If your child needs to their inhaler in their backpack while on campus, the parent must sign a Self-Carry (Consent to Carry) form at the Health Center.

    If your child will need a nebulizer treatment while at school, please bring the medication vials in the original box with prescription label on the outside and nebulizer tubing with you.  The Health Center has a nebulizer machine but tubing cannot be shared among students.

    **We will need the original prescription box for any asthma inhaler or nebulizer treatment.**

    Diabetes

    If your student has diabetes, please complete a Diabetes Medical Management Plan and return to the Nurse before the school year begins. Any necessary maintenance supplies may be kept in the Nurse’s office such as:

    • Lancing devices, lancets
    • Insulin pen, needles, insulin cartridges
    • Insulin pump supplies and batteries
    • Insulin vials and syringes
    • Ketone strips
    • Quick Carb resources, other snacks, juice boxes

    Refrigeration and needle disposal are available in the Health Center.

     

    Seizures

    If your student has a history of seizures or a seizure condition, please complete a Student Seizure Information Sheet and bring any necessary medication along with a completed Medication Log to the Health Center prior to school entry.
