Here are some tips to prepare for returning to school:
Are your child's immunizations up to date? If in question please speak to your health care provider. By age 5 children are required by AZ state law to have for school: 5 DPT's, 4 Polio's (IPV), 3 hepatitis, 2 MMR's, and 1 varicella. By age 12 they are required to have a Tdap and Meningitis immunization. Children are not allowed to attend school without proper immunizations, per AZ state law.
Prescription Medication: must be in pharmacy-labeled packaging, brought to the front office BY A PARENT, and accompanied by a "Medical Authorization to Administer Medication" form SIGNED BY A PHYSICIAN. This can be found in the "Forms" section of this webpage. No prescription medication will be given at school without this completed form.
Over the counter Medication: must be brought to the front office BY A PARENT in its original packaging/bottle. This includes Tylenol, Ibuprofen, Cough Drops, Allergy medication, Antacids, Neosporin. A Medical Authorization to Administer Medication form will be completed by the parent and nurse with medication drop off.
Chronic health conditions: If your child has a chronic health condition, for example: anaphylactic allergy, diabetes, asthma, or seizure disorder, please find the appropriate forms on the "Forms" page of this webpage and have them completed before the start of school. Some of these forms need to be filled out by a physician. All of these forms are required to be renewed each school year online.
Hearing and vision screening will be conducted within the first few months of school on all new-to-Diamond-Canyon students as well as Kinder, First, Third, Fifth, Seventh and Special Education students. Parents/guardians will be notified of any atypical results.
